1. I am at

jurisdiction. The material I am requesting is for my own personal use and will not
to be used and/or viewed by minors or anyone else. I understand and agree that the
materials will NOT be used or redistributed in any form or manner under penalty of
prosecution by law. I understand that "Miller" retains all copyrights to
the materials and may resell or redistribute as deemed appropriate.
2. I am not
a Law Enforcement Agent of any kind, a US Postal Official, or another, attempting to
obtain any evidence for the prosecution of any individual, entity or Corporation or for
the purpose of entrapment.
3. I agree
that the material that I'm ordering is not obscene or offensive to me and I enjoy
using/viewing sexually explicit material for personal purposes. I am also certifying that
the erotic and/or sexually explicit material I'm ordering is not illegal or considered
obscene in my community. I hereby certify under penalty of perjury that all of the

4. I
understand that all sales are final and no refunds, exchanges and/or credits will be
considered unless proof of shipping damage is provided to "Miller" within 48
hours of delivery.
5. I hereby
release "Miller" from any and all obligation, responsibility and/or liability
related to this transaction or the materials forwarded to the Shipping Address below.
6. By

line below constitutes signing a legal, binding document just as if I had printed the
